Director of Intake and Service Navigation Essential Responsibilities: 

  • Lead and continuously improve Momentum’s centralized intake and service-navigation process from referral through admission and post-admission handoff.
  • Oversee complex and high-risk referrals, serving as the primary escalation point for urgent, clinical, cross-regional, and challenging placements.
  • Coordinate referral and admission reviews, ensuring required medical, behavioral, financial, benefits, funding, and program documentation is complete and supports informed placement decisions.
  • Coordinate pre-admission assessments and placement activities, including in-home, hospital, skilled nursing, program-site, and community-based assessments. Travel throughout Momentum’s service area is required.
  • Partner with nursing and clinical teams to assess medical readiness, level of care, behavioral needs, accessibility, equipment, transportation, staffing, and other supports required for successful placement.
  • Evaluate admission readiness and placement options, recommending appropriate services, alternative placements, delayed admissions, or other solutions based on individual needs and program capacity.
  • Provide ongoing service navigation for individuals and families, helping them access appropriate services across Momentum’s residential, adult day, pediatric therapy, ABA, and community-based programs as their needs change.
  • Monitor referral pipelines, census, capacity, vacancies, and admission timelines, identifying barriers and opportunities to improve occupancy and service access.
  • Collaborate with Regional Centers, payers, Finance, Billing, and program leadership to confirm authorizations, funding, rates, service requirements, and financial readiness before services begin.
  • Lead cross-functional intake and placement efforts while ensuring compliance, accurate documentation, effective handoffs, and a positive experience for individuals and families throughout the intake process.

Qualifications: 

  • Bachelor's degree in healthcare administration, social services, business, public administration, or a related field; required. 
  • Minimum of five years of progressively responsible experience in intake, admissions, service coordination, healthcare or disability-services operations, case management, or program administration.
  • Minimum of three years of direct supervisory or healthcare leadership experience, including responsibility for assigning work, coaching staff, evaluating performance, addressing performance concerns, and developing an effective team. 
  • Experience with developmental-disability services, Regional Centers, residential or adult day programs, healthcare transitions, service authorizations, and interdisciplinary teams strongly preferred.
  • Experience using data to manage census, capacity, referral pipelines, operational performance, and financial readiness.
  • Current and valid California driver license, liability insurance, and reliable means of transportation; required.
  • Must be available to travel throughout Los Angeles, Ventura and Orange Counties, as needed (mileage reimbursed)
  • Successfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, and provide proof of eligibility to work in the United States
  • Ability to run, push, pull, bend, squat, kneel, and lift up to 35 lbs.
