Community Developer - Adult Day Program
*This position will temporarily be located at our Chatsworth location with plans to move to Thousand Oaks Spring 2024!
Momentum is seeking a Community Advisor for our new Adult Day Program in Thousand Oaks. The Community Advisor will promote, develop, and facilitate opportunities for community integration and access, both remotely, in the home studio setting, and at places the individual chooses to spend their time. The Community Advisor will support, instruction, and assistance with a variety of activities chosen by the person receiving services.
Community Advisor Essential Responsibilities:
- Collaborates with Program Management Team to develop an individualized curriculum to meet the physical needs and activities for each individual attending specific class
- Collaborates with the Program Manager and Art Advisors to develop individual activities and goals
- Provides and organizes schedules, support, and instruction.
- Develops related long-range goals and learning objectives for individuals in conjunction with Art Advisors
- Generates curriculum supply needs and submit requests to Program Manager
- Provides any level of direct support required by program participants, including but not limited to, personal care in the restroom, assistance to eat and drink, and coaching individualized instruction
Education and Experience:
- High school diploma or equivalent; required
- Some college level courses in human services; preferred
- Minimum of two (2) years of experience working with people with developmental disabilities in a service or educational setting
Skills and Knowledge:
- Ability to assist with personal care needs of individuals; including restroom support and feeding
- Ability to work in a professional manner with other departments within the facility and with community representatives
- Ability to effectively communicate and follow directions, both orally and written in English
- Positive attitude with flexibility to meet the needs and demands of the department
- Ability to reorganize workload and direct staff to assure completion of all personal care and program goals
Requirements:
- Must have a valid California driver’s license, insurance, reliable transportation, and clean driving record
- Ability to run, push, pull, bend, squat, kneel, and lift up to fifty (50) pounds
- Successfully pass all post-offer, pre-employment screening requirements, including criminal background check and provide proof of eligibility to work in the United States
